HOW TO JOIN US
If you are interested in joining the Learning Disability Support Centre, please complete the referral form below and somebody will be in touch with you.
You must be over 18.
We will contact you to arrange a face-to-face assessment within 5 working days.
By completing the form, you consent to us recording your details in order to make an assessment.
If you would like to find out more about the Centre, or you are unable to complete the form then please look on our Contact Us page to find alternative ways to speak to us, we can arrange your referral over the phone if preferred.
Please note the new LDSC has limited spaces, please book an
assessment to avoid disappointment.
